Gilead posts positive results in mid-stage Hepatitis C study

(Reuters) - Gilead Sciences Inc on Saturday reported a 100 percent cure rate using a combination of drugs in a small number of patients with the most common and hardest to treat form of hepatitis C.

Rival Abbott Laboratories Inc, meanwhile, said a trio of its oral medicines to treat hepatitis C produced unprecedented cure rates in a larger number of patients who had failed to benefit from standard treatment, as well as very high cure rates for newly treated patients.

Data from both companies' mid-stage trials were released Saturday at the annual meeting of the American Association for the Study of Liver Disease in Boston.

Gilead's study, dubbed Electron, examined 25 patients with genotype 1 chronic hepatitis C virus (HCV) infection who were treated for 12 weeks with a combination of three drugs: sofosbuvir, ribavirin and GS-5885.

GS-5885 is from a promising new class of drugs known as NS5A inhibitors, which prevent the hepatitis C virus from replicating.

The infection was undetectable four weeks after completing therapy in all of the patients who had never received this combination of drugs before, Gilead said.

The drugs generally were well tolerated in the study, Gilead said.

In the sofosbuvir combined with GS-5885 and ribavirin patient groups, one patient dropped out because of an adverse side effect that the company said was unrelated to the drugs.

Sofosbuvir and GS-5885 are still being studied for their safety and efficacy.

The biopharmaceutical company will present the data on Tuesday at the annual meeting of the American Association for the Study of Liver Diseases in Boston.

Mark Schoenebaum, a biotech analyst with ISI Group, said in a research note that he expects Gilead shares to rise on Monday based on these "best case" results.

UBS analyst Matthew Roden said "these data strongly support Gilead's leadership position" in the hepatitis C virus space.

Gilead recently started the first Phase 3 trial evaluating a fixed-dose combination of sofosbuvir and GS-5885 in patients with genotype 1 chronic hepatitis C virus infection who had not received these drugs before.

This study is evaluating the fixed-dose combination with and without ribavirin for 12- and 24-weeks in 800 patients, 20 percent of whom have evidence of cirrhosis, or liver scarring.

Vatican computer tech convicted in leaks scandal

VATICAN CITY (AP) ? A Vatican court has convicted a Holy See computer technician of helping the former papal butler in the theft of confidential papal documents and given him a two-month suspended sentence.

Claudio Sciarpelletti, an Italian who is a computer program analyst in the Vatican's Secretariat of State, had testified earlier Saturday that he had played no role in helping to spirit out confidential documents in a scandal involving alleged corruption in the Vatican bureaucracy.

Pope's former butler, Paolo Gabriele, was convicted last month in a separate trial for the theft of the documents and is serving a 18-month prison sentence in Vatican City.

Top Vatican security officials, including the head of Pope Benedict XVI's bodyguards, as well as his convicted former butler are on the witness list in the latest trial in the leak of confidential papal correspondence.

The witnesses had been called to testify earlier in the week in a Holy See courtroom, but the judge told them to come back Saturday to give more preparation time for the defense.

Vatican computer programmer Claudio Sciarpelletti is on trial on suspicion he aided ex-butler Paolo Gabriele, who is serving an 18-month prison sentence for the theft of the documents.

A pool of eight journalists was chosen to cover the proceedings, which were to begin Saturday morning.

The stolen documents formed the basis of an Italian journalist's book about alleged corruption at the Vatican.

Boras: Dodgers 'bought store,' Mets 'in freezer'

INDIAN WELLS, Calif. (AP) ? With baseball awash in record revenue as the signing season starts, Scott Boras compares the habits of teams to families sifting through supermarket shelves.

At the winter meetings in Dallas last year, the agent had this to say of the financially troubled Los Angeles Dodgers and New York Mets: "Normally, they're in the steaks section, and I found them in the fruits-and-nuts category a lot."

Since then, the Dodgers have been sold for $2 billion. The Mets owners have agreed to pay up to $162 million ? and likely much less ? in a deal with the trustee for Bernard Madoff's fraud victims.

So on Wednesday at the general managers' meeting, Boras said his view of the Dodgers had changed.

"I think they bought the store," he said.

And as for the Mets?

"The best you can say is that they might be in the freezer section," he explained. "But there's a lot of good, longstanding products that they can acquire there."

Slugger Josh Hamilton and pitcher Zack Greinke are among the top players in a relatively weak free-agent class that also includes outfielders B.J. Upton, Michael Bourn, Torii Hunter and Nick Swisher; first baseman Adam LaRoche; and pitchers Kyle Lohse and Rafael Soriano.

Baseball estimates revenue this year at $2.5 billion ? an increase of about $500 million. National television contracts with Fox and Turner that run from 2014-21 will double the average yearly money baseball receives to about $800 million.

And perhaps the biggest evidence of baseball's wealth is franchise values ? the Dodgers sold for $2 billion this year in a bankruptcy court auction and the lowly San Diego Padres were bought for $800 million.

"So now we've had owners that have become instant billionaires. They've made over a billion dollars by this reevaluation," Boras said. "I don't think we need Forbes anymore to tell us what franchises are worth because we had a bankruptcy court judge say this is open season, come tell me what this is all worth, and I think we got a proper accounting. Franchise values are through the roof because interestingly enough, we know what a near-top team is worth and we know what a near-bottom-value team on the market is worth by what's gone in the past three or four month. So I think there's a lot of smiling faces for people who invested in this industry in the early 2000s."

Since the Dodgers were bought last spring by a group headed by Mark Walter, Stan Kasten and Magic Johnson, they increased their payroll by about $35 million, adding infielder Hanley Ramirez, first baseman Adrian Gonzalez, closer Brandon League, pitcher Josh Beckett and outfielder Carl Crawford.

"Today compared to a year ago, we're in much better shape," general manager Ned Colletti said.

The Dodgers' payroll next year might approach or surpass that of the New York Yankees, who have topped spending every year since Los Angeles edged them by $1 million in 2001.

New York is vowing to cut its payroll by 2014 because of changes in the collective bargaining agreement that will cut the team's revenue-sharing bill if it doesn't wind up paying a luxury tax.

"Every year you're kind of given your budgets, your challenges, your needs," Yankees general manager Brian Cashman said. "You always have your restrictions, whether it's in terms of what ownership provides, the Basic Agreement allows."

It is in Boras' interest to encourage spending. His free-agent group includes Soriano and fellow closers Jose Valverde and Ryan Madson, first baseman Carlos Pena, Bourn, Lohse and pitcher Daisuke Matsuzaka.

Infielder Maicer Izturis became the first major league free agent to switch teams this offseason, agreeing to a $10 million, three-year contract with the Toronto Blue Jays. The 32-year-old hit .256 with 17 steals in 19 tries, two homers and 20 RBIs this year for the Angels.

"He's someone that's going to come in and compete for the job at second base," said Blue Jays general manager Alex Anthopoulos, who tried to acquire him last summer. "He's not guaranteed the job, but the fact that he can play all over the infield, he's just a valuable guy to have. He can do a lot of little things. He's a winning player. He can draw a walk. He can put the ball in play ? a pretty good contact rate. He can run a little bit and steal some bags. He plays very good defensively, as well, and is a good teammate, good in the clubhouse."

As the meetings conclude Friday, the deadline will pass for free agents to decide whether to accept $13.3 million qualifying offers from their former clubs. Among the eight players on the market who were given the offers, pitcher Hiroki Kuroda appears to be the only one who might accept. He is coming off his first season with the Yankees.

MMObility: Windows 8 and gaming in the Metro browser | Massively

So, you've heard about the recent Windows 8 release but aren't sure whether or not you should upgrade? How will the operating system affect your gaming, and are you going to go with any of the Microsoft Surface products? All of these questions can be answered in numerous ways, but I wanted to answer it from the standpoint of a mobile gaming fan. After all, it is my belief that mobile gaming will soon dominate gaming in the world... might as well be prepared!

But how does Windows 8 affect a mobile MMO fan like yours truly? Well, Windows 8 does a heckuva lot. It offers a ton of flexibility and fun new ways to interact with your PC. But it can also be a very confusing new addition. I sat down to figure out some of these issues with the help of Ben Adams, CTO of Illyriad Games. He knows more about mobile tech than I do and was able to sort out the differences between the Windows 8 browsers for me. Let's get to it.

Windows 8 really shines on a touch-enabled device. I myself own a Dell Inspiron Duo, a netbook that transforms into a tablet. You might have seen other laptops or netbooks attempt this, especially recently. Well, the Duo is a great device but is sort of underpowered for hosting a full copy of Windows 7 as it did when it shipped. I didn't have as much fun with it simply because the full Windows 7 experience was clunky and slow, even though it was handy when I needed a full PC that weighed a few pounds.

I started by adding the Windows 8 preview and later upgraded to the full Pro version as soon as I could. It's only around $40 US, so it's a great deal; a household could easily afford copies for all machines. Immediately the new OS made life with the formerly sluggish device much better. Performance went up, and the emphasis on touch meant that I actually used the little netbook in the way it was intended. If I need access to a full browser to play, say, Glitch, I can have it. If I prefer to use the new "Metro" version of Internet Explorer 10 (which is much faster and better for touchscreens), then I can do that as well.

Unfortunately, this means that I have to use a few different browsers on any one day. Allow me to explain.

Windows 8 comes with essentially two browsers, both versions of the same Internet Explorer, but one is the full, normal Internet Explorer experience that you might (or might not) be used to, while the other is the Metro version. The Metro version is a lighter, faster, and generally safer browser that is suited perfectly for touch. It features large buttons when they are needed, but when in general use, the browser stays in glorious fullscreen mode. Right-clicking brings up any menu I need. I actually prefer it over other browsers -- that is, until I need a full browser experience.

See, Metro does away with a lot of the browser addons you might use. It includes Flash, but only from sites that have been "whitelisted" by Microsoft. That means a game like Glitch cannot run in the Metro browser. In fact, Metro does away with a lot of standard plugins that can actually make our normal browsing slower and in a lot of cases less secure. Many security issues stem from common plugins like Flash and Java, but Windows often gets the blame for those issues. With Metro, Microsoft has now avoided being listed on Kasperky's list of top 10 vulnerabilities. The article even recommends uninstalling Java completely! (It's not allowed in the Metro browser.)

So, without all of these common plugins, what is a mobile gamer to do with a Metro browser? Your best bet is to go for HTML5 games. I have a list of these games bookmarked on all of my machines. The list includes Illyriad (already in the Windows 8 store), Grepolis, Command and Conquer: Tiberium Alliances, Lagoonia, and others. The great thing about HTML5, and the reason Google, Microsoft, Adobe and others are paying such attention to it, is that it will work in almost anything. Build it in HTML5 and you can do away with many incompatibilities. You can run whitelisted Flash-based MMOs on the desktop version of IE on the Surface RT.

Of course, you can also wait until the other Surface is released, the Pro version. It comes with a 3rd-gen Intel Core i5 processor and four gigs of RAM. It's powerful enough to handle many browser-based MMOs, even some of the heavy hitters that utilize fancy graphics or engines like Unity. There are a lot of choices with games like Glitch, Forge of Empires, Nadirim, Ministry of War, RuneScape and many, many others. The Surface Pro is basically a laptop or a very powerful tablet. Will it be worth the cost?

All of those Flash-based MMOs like Evony or those hosted on Facebook should be able to run with no issues on the Surface Pro desktop browsers. Be aware that some games, like Dungeon Blitz on Facebook, will still require a keyboard. Luckily you can nab one of the nifty keyboard covers with your Surface, or you can use an add-on.

It might seem odd that Windows 8 ships with the two different browsers. But let's remember what Microsoft is attempting to do. The company is moving into the modern era with an OS that actually works well with touch. Touch-enabled devices, like smartphones and tablets, will likely take over the market as primary personal computers for most people. If we want to use a faster, safer browser for surfing the web and performing basic tasks as well as play HTML5 and other "whitelisted" games, then we can use the Metro browser. If we need to use a standard browser, however, we will still have the choice.

Remember what Apple has done with the iPad. To date, Apple has sold millions and millions of the nifty devices and has literally changed the way the market expects to compute. Apple essentially told consumers, "Buy our way or don't use the iPad," and now many of us are comfortable using the app store. Microsoft is moving things forward by emphasizing HTML5, a more flexible medium, but still allowing consumers to find the same browser experience as before. Having choices is always better than having none.

I am loving Windows 8 on all of my devices and actually prefer the Metro browser so much that it is my default browser. If I open a link or bookmarked site that demands Flash, like Glitch, then it opens the appropriate desktop browser. In exchange I have a more solid and much faster operating system that has breathed new life into my older, touch-enabled netbook and even into my brand-new desktop.

On the Downward Spiral: The Kindling Effect of Addiction

The kindling effect of addiction is not only responsible for repeated relapse events in addicts and alcoholics; it also makes each withdrawal more severe and potentially dangerous than the last. Unfortunately, there is still a significant amount of resistance to concepts centered on drug abuse such as the disease model of addiction, post acute withdrawal and the kindling effect.

However, considering that the kindling effect is accepted in many other areas of medicine, it?s logical to argue that this condition must be addressed pertaining to substance abuse if there is to be any hope of mitigating the serious public health threat of addiction.

Sensitized Versus Desensitized

To kindle a fire means to stoke it: to make it burn brighter, higher and longer. In medicine the idea of ?kindling? is essentially the same, but can occur in one of two ways: either making the ?fire? hotter, or dimming or extinguishing the fire completely. Consider the following scenarios:

  • Sensitized: A person who uses a specific drug for a long period of time ? especially alcohol ? may become more sensitive to it as a result of the body?s dwindling ability to process the substance.
  • Desensitized: A person who uses a drug consistently may become desensitized to it, as in the case of tolerance, where more and more of the substance is required to produce the same effect or ?high.?

The kindling effect can also have a significant impact on a person?s behavior, irrespective of substance abuse or alcoholism. Consider the story of a young boy reported in PsychCentral.com by Nanette Burton Mongelluzzo. From an early age the boy was subjected to a series of accidents, then torment by his peers later as a result. Each progressive incidence caused the boy to become highly sensitive to certain events and interactions, until the point where he developed severe anxiety and what could only be termed as ?hyper-vigilance.?

Addicts and alcoholics suffer from the kindling effect in a number of ways, including sensitization or desensitization to a substance, as well as behavioral kindling related to their drug use. But for people attempting to get clean, kindling has a number of other effects that can actually make withdrawal more dangerous and less likely to succeed.

Physiological Kindling

The process of becoming addicted to a substance causes changes in neuronal activity in the brain and creates neurological pathways that ?service? the addiction. With sudden cessation of drug use or drinking come a ?reversal? of these changes, whereby neurons become hyperactive after being suppressed during active addiction. The kindling effect occurs with each successive relapse and withdrawal event.

As neurons change and adapt to active addiction and subsequent clean periods, they eventually become more and more sensitive each time withdrawal occurs. This is evidenced by an increasing severity and duration of symptoms related to AWS or Acute Withdrawal Syndrome. Dr. Howard C. Becker ? a noted expert on alcoholism ? explains in his article ?Kindling in Alcohol Withdrawal?:

?Accordingly, although binge drinking may not initially result in serious, or even noticeable, withdrawal symptoms, repeated episodes of this pattern of alcohol intoxication followed by abstinence and withdrawal may lead to a worsening of future withdrawal-related symptoms.? ?(http://pubs.niaaa.nih.gov/publications/arh22-1/25-34.pdf)

And because it is these very symptoms that cause addicts to relapse in an effort to seek relief, the physiological effect of kindling in addiction is a significant risk to people in recovery. This is true for people of all ages, including teens and young people.

Behavioral Kindling

Behavioral kindling related to addiction is similar to the story with the young boy above. By continually going through active periods of substance abuse and then periods of sobriety, a person can become overly sensitive to living with ? or without ? their drug of choice. Relapse becomes more likely because stimuli that would cause little reaction in most people causes an extreme and compelling need to seek solace in substance abuse in the addicted individual.

Once the kindling effect has set in, it can pose significant challenges to the recovery of the affected person. This is often evidenced by the behavior of addicts or alcoholics that try time and time again to get clean with no results except the downward spiral of destroyed relationships, careers and health. In fact, the ?revolving doors? of most drug detox centers and rehabs swing on the hinges of the kindling effect.

What this means in lay terms is that although many addiction experts expound the idea that relapse is a normal part of addiction, it?s actually a progressive, severe problem that must be arrested permanently, as early as possible. This requires educational programs, facilities, resources and infrastructure that subscribes to the notion of;

?You must stop using now and forever,? instead of ?Better luck next time, see you when you get back.?

If we look at addiction and alcoholism as the chronic and potentially fatal diseases they are, it?s easy to see that the kindling effect is equivalent to a remission of cancer, whereby each new recurrence could be the final, fatal relapse. And because addiction is our most serious public health issue, spreading the word about this condition is essential considering its relative obscurity.

Ultimately, we?ve got to start a fire about the Kindling Effect.
